I give permission for school administration to share and exchange other information with staff at Freedom Apex Solutions and/or other affiliations/agencies for the purposes of mentorship, academic development, diversity inclusion and financial literacy.

I understand that the information shared could include:

• IEP
• 504 Plans
• Psychological Report
• DRS case status information
• Physical
• Academic report card

This consent is valid for one year from the date of the signature. I understand that I may withdraw, in writing, at any time.
